ATHLETIC PHYSICAL INFORMATION:
All students participating must have a physical form completed before they can compete in any practice, scrimmage or game.
Physicals at Holy Name are offered at two times during the year. One time on or after June 1, at the end of the school year and one time in late October or early November. Physicals dated after June 1, 2010 will be good for the entire 2010-2011 School year.
These physicals are free to all Holy Name Athletes.
There is a new PIAA form for athletic physical examinations and a slightly different recertification process affecting PIAA interscholastic athletic programs grades 7-12.
This is now the ONLY form acceptable for participation in athletics.
Students are now required to obtain a physical evaluation for a given school year after June 1 of the upcoming school year. The physical exam will remain valid through the end of the school year barring major injury or illness. (See below for major injury/illness information.)
The new form for the athletic physical is called the Comprehensive Initial Pre-Participation Physical Evaluation or CIPPE.
There are four sections to the CIPPE:
· Sections One, Two and Three must be completed by the parent/guardian and student.
· Section Four must be completed by a licensed physician of medicine (M.D.) or osteopathic medicine (D.O.).
· All sections must be returned to the Holy Name Athletic Office where it is kept on file.
RE-CERTIFICATION for Students for Winter & Spring Seasons
NO major injury / illness
Students who have participated in the fall season who wish to participate in the winter and/or spring seasons do not require a new physical exam. However, a student and the parent/guardian must complete a re-certification for each sport in which he/she wishes to participate. Barring any major injury/illness since the physical exam, simply complete Section 5 of the CIPPE form and return it to your school.

Major injury / illness
In the event the student has suffered a major injury
or illness since the last physical exam:
· Section 5 must be completed by the parent/guardian.
AND
· Section 6 must be completed by a licensed physician
of medicine or osteopathic medicine.
